Job Title:
Physical Therapist

Description:
Plans and administers medically prescribed physical therapy treatments for patients.

Essential Responsibilities:
1. Reviews physician’s referral and all available medical records pertaining to the patient’s condition.
2. Evaluates the neuro-muscular-skeletal systems as they pertain to the patient’s referral for therapy.
3. Records the evaluation and designs a treatment program with goals set appropriately for the diagnosis.
4. Provides treatment of adolescent, adult and geriatric patients, adapting the treatment plan as needed.
5. Provides on-going treatment and re-evaluation of the response to the treatment as they relate to the goals.
6. Utilizes appropriate manual skills and physical agents to realize the goals set for the patient.
7. Documents on-going progress towards reaching goals and appropriate changes in the neuro-muscular-skeletal systems.
8. Instructs patient and family in home programs.
9. Evaluates, fits, and adjust orthotic devices and communicates recommendations to the orthotist.
10. Prescribes essential take-home equipment for the patient.
11. Participates in discharge planning, including home checkouts and recommended home modifications
12. Follows all departmental and hospital policies and procedures.
13. Monitors programs assigned to the physical therapy aides for his/her patients.
14. Communicates on an on-going basis with the physician and other team members in regard to patient programs.
15. Performs requests as outlined by the P.T. Supervisor or the Director of Physical Therapy.
16. Follows hospital infection control policies, including body substance isolation and proper hand hygiene before and after contact with patients, handling of food, and after hands are contaminated with any body fluids, secretions or excretions.
17. Understands his/her responsibility to inform patients, family and visitors of the importance of hand hygiene in all environments.

Other Job Duties and Responsibilities:

1. Proper check out/in and storage of equipment.
2. Clinical instructor for physical therapy students.
3. Shares minor housekeeping duties.
4. Manages office files, supplies, and communications sheets
5. Provides on-going education for other disciplines.
6. Serves on hospital committees
7. Organizes interdisciplinary groups and outings.

Requirements:
Education - Graduate of a physical therapy curriculum accredited by the American Physical Therapy Association.
Experience - Two years experience in the field of physical therapy preferred.

5. Follows written and oral instructions

Abilities:
1. Determining time, place, and sequence of operations or action to be taken on the basis of analysis of data; executing determinations and/or reporting on events.
2. Teaching subject matter to others, or training others through explanation, demonstration, and supervised practice; or making recommendations on the basis of technical disciplines.
3. Using body mechanics and/or tools or work aids to work, move, guide, or place objects or materials in situations where ultimate responsibility for the attainment of standards occurs and selection of appropriate tools, objects, or materials and the adjustment of the tool to the task require exercise of considerable judgment.

Other Qualifications:
1. Current Colorado physical therapy license or licensed in another state and application made for Colorado license
2. Current CPR

Schedule:
Per-Dim/ On-Call
