 Interactive SCI Health Assessment
Welcome to the Craig Hospital Wellness
and Risk Assessment
Profile (WRAP). This assessment tool has been developed especially
for persons living with spinal cord injury (SCI) to review lifestyle
behaviors and provide recommendations for healthy choices. You will
be asked to respond to the following areas:
- Personal Information
- Smoking
- Physical Activity
- Stress Management
- Safety
- Alcohol/Drug Use
- Self-Care Exams
The WRAP can only provide general recommendations__you
must take into account your individual health status, finances, and
community resources. The recommendations provided to you at the
end of the Wellness Assessment Profile do not take the place of
consulting with your physician or other health care providers.
To begin, please click the continue button. You will be asked
a series of questions about yourself in general. After you've
completed those questions, click on the "Select Wellness Section"
button. You then will have seven options for wellness recommendations.
